About Your First Visit and Initial Visit Paperwork

I look forward to seeing you at your first visit to Elements Acupuncture and Wellness.


For your convenience, you can access the Initial Visit Paperwork HERE so that you can download and print the paperwork ahead of time. If you would prefer to fill out the paperwork at my office, simply arrive 10 – 15 minutes before your appointment time to do so.


Please remember to bring your insurance card if you are billing insurance.


Elements Acupuncture and Wellness is located in the South Eugene Wellness Center. There is ample free parking and the building is fully ADA compliant. Elements reception is located in Suite 100, straight ahead of you as you enter through the front door. It is advantageous to be well-hydrated before your appointment and to wear loose and comfortable clothing, if possible.
